PowerPoint for a report which will be attached

i will attach a report which the presentation slide power point will be about with the course profile to know the instructions.in the course profile its page 8 i need a 5 sildes because its a short presentation 15 mintues only.and please i want the report and presentation in a one file can you please do it.
